摘要：

目的分析中国40岁以上人群原发性骨关节炎的患病状况。方法按全国行政区划系统, 选择六个流调中心:石家庄市、哈尔滨市、上海市、广州市、成都市和西安市。调查对象为年龄40岁及以上汉族男女一般人群。按分层、多阶段、整群随机抽样的原则进行抽样, 样本数均为1 000人, 城乡各半、男女各半。原发性骨关节炎诊断标准采用美国风湿病协会1995年修订的诊断标准, 调查部位为双膝、双手、颈椎和腰椎。结果共完成调查表6 218份, 获取X线资料5 334份。六个流调中心40岁以上人群原发性骨关节炎的总体患病率为46.3%(2 877/6 218);男性为41.6%(1 212/2916), 女性为50.4%(1 665/3 302)。不同年龄段原发性骨关节炎患病率分别为:40～49岁患病率30.1%(646/2 145), 50～59岁患病率48.7%(1 086/2 230), 60～69岁患病率62.2%(754/1 213), 70岁以上患病率62.1%(391/630)。城乡原发性骨关节炎患病率分别为:城市45.6%(1 472/3 225), 农村46.9%(1 405/2 993)。不同部位患病率分别为:颈椎23.6%(1 468/6 218), 腰椎29.4%(1 829/6 218), 双膝15.6%(972/6 218), 双手7.8%(484/6 218)。结论基于六大行政区划的中国40岁以上人群原发性骨关节炎总体患病率为46.3%, 呈现随年龄增长而增高的趋势。女性患病率高于男性, 城市与农村人口患病率无差异。
