缺血性卒中患者血管周围间隙扩大与卒中后抑郁的相关性

摘要
目的探讨急性缺血性卒中患者血管周围间隙扩大(enlarged perivascular space, EPVS)与卒中后抑郁(post-stroke depression, PSD)的相关性。方法前瞻性连续入组2010年3月至2014年3月期间住院治疗的急性脑梗死患者, 入院后完成头颅MRI检查并进行EPVS分级, 在发病3个月时根据汉密尔顿抑郁量表(Hamilton Depression Scale, HAMD)和《中国精神障碍分类与诊断标准(第3版)》进行PSD评定, 分析EPVS与PSD之间的联系。结果共入组249例患者, 其中62例(4.9%)在3个月时发生PSD。PSD组基线美国国立卫生研究院卒中量表评分[中位数(四分位数间距);4(3~6)分对3(2~5)分;Z=–2.950, P=0.003]以及半卵圆中心(χ2=14.370, P=0.001)和侧脑室旁(χ2=11.590, P=0.003)EPVS分级患者构成比与非PSD组差异存在统计学意义。多变量logistic回归分析显示, 半卵圆中心EPVS2级[优势比(odd ratio,OR)3.89, 95%可信区间(confidence interval,CI)1.59~9.56]和3级(OR 3.28, 95%CI 1.04~10.33)与PSD显著相关;侧脑室旁EPVS 2级(OR 0.72, 95%CI 0.27~1.91)和3级(OR 2.24, 95%CI 0.68~7.37)与PSD无关。结论半卵圆中心EPVS与PSD独立相关, 而侧脑室旁EPVS则不然。
