基于OPG/RANKL信号轴探讨二至丸治疗绝经后骨质疏松的作用机制

摘要
目的:探讨二至丸治疗绝经后骨质疏松模型鼠的作用机制。方法:将108只雌性SD鼠随机分为假手术(Sham)组、模型(PMOP)组、雌激素治疗(E2)组、二至丸低剂量治疗(EZW-L)组、二至丸中剂量治疗(EZW-M)组、二至丸高剂量治疗(EZW-H)组,每组18只。E2组接受E2针剂皮下注射,EZW-L组、EZW-M组、EZW-H组分别予以6 g/kg、9 g/kg、12 g/kg混悬液灌胃,Sham组与PMOP组均予以1 mL生理盐水灌胃,均干预12周。检测各组大鼠右侧股骨头骨密度(BMD)、股骨生物力学/病理变化以及骨代谢血清标志物BALP、TRACP-5b、Cath-K、OPG、RANK、RANKL的水平变化。结果:药物干预12周后,与PMOP组比较,EZW-M组、E2组、EZW-H组的BMD、弹性模量、最大载荷、BV/TV、Tb.Th、Joint、血清骨代谢标志物BALP、TRACP-5b、Cath-K的蛋白表达均显著增高(均P<0.05),FLAW显著降低(P<0.05);HE染色显示PMOP组骨组织出现明显病理改变,E2组和EZW-H组疏松病理现象改善明显。POMP组、EZW-L组、EZW-M组股骨中OPG蛋白含量较Sham组、E2组、EZW-H组明显下降;RANK、RANKL蛋白含量升高。与PMOP组比较,EZW-M组的OPG/RANKL比值显著增高(P<0.05),RANK/RANKL比值显著降低(P<0.05),E2组、EZW-H组的OPG/RANKL比值显著增高(P<0.01),E2组、EZW-H组的RANK/RANKL比值显著降低(P<0.01)。结论:二至丸可改善骨质疏松,其作用机制可能是OPG骨保护素重组蛋白/核因子κB受体激活物配体/RANKL信号轴抑制破骨细胞成熟分化实现的。
