维生素D缺乏与感染发生的相关性及其机制研究进展

摘要
目的:认识维生素D在抗感染方面的作用,了解维生素D参与抗感染作用的可能机制。方法:以"维生素D""1,25-二羟基维生素D""25-羟基维生素D""骨化三醇""感染""炎症""抗菌肽""防御素""免疫""一氧化氮""LL-37""Vitamin D""Calcitriol""Cholecalciferol""Calciferol""25-hydroxyvitamin D""1,25-dihydroxyvitamin D""Infection""Anti-inflammation""Cathelicidin""HBD""HNP""Immunity""Nitric oxide"等为关键词,组合查询1997-2017年在中国知网、Pub Med、Web of Science等数据库中的相关文献,对维生素D与感染相关性的研究现状及维生素D参与增强抗感染作用的可能机制进行综述。结果与结论:共检索到相关文献500多篇,其中有效文献46篇。维生素D缺乏的人群更易发生感染,而补充维生素D具有一定的辅助抗感染作用,主要包括急性呼吸道感染、皮肤软组织感染及脓毒血症、细菌性阴道炎等。维生素D可能通过促进抗菌肽的产生、上调维生素D受体的m RNA表达、影响免疫调节、影响细菌细胞膜通透性来发挥辅助抗感染作用。目前对维生素D抗感染机制的研究多处于体外试验水平,在体内的研究深入不够;而在维生素D抗感染作用方面,仍缺乏大规模临床研究数据。今后仍需要大规模的临床试验以进一步证实维生素D的抗感染作用,并探寻给药方式及剂量,以期能够作为临床抗感染的辅助治疗或预防用药的手段。
