可乐定对罗哌卡因蛛网膜下腔阻滞效应和血液动力学影响的观察

摘要：

目的　观察小剂量 (5 0 μg)可乐定对罗哌卡因 (Rop)蛛网膜下腔阻滞效应和血液动力学的影响。方法　 4 0例择期下肢手术病人随机分为四组 :B组 (n =10 )布比卡因 (Bup) 10mg、BCLO组 (n=10 )Bup10mg +可乐定 5 0 μg、R组 (n =10 )Rop10mg和RCLO组 (n =10 )Rop10mg +可乐定 5 0 μg。每组药量配成等比重液 2ml。测定腰麻给药后 0、5、10、15、2 0、2 5、30、4 5、6 0、90、12 0min或术毕的无痛平面、最大阻滞节段、Bromage运动评分、腰麻后痛觉出现时间和Bromage评分≥ 1的维持时间 ,并测定MAP、HR ,和连续阻抗法的CO、SV等血液动力学指标 ,并观察出现的副反应。结果　各组麻醉效果均较满意 ;各组MAP、HR、CO、SV等均无显著性变化 (P >0 0 5 ) ;最大阻滞节段及其出现时间均相似 ;RCLO组比R组的痛觉阻滞时间 (2 97 0minvs 2 4 5 8min)和运动阻滞时间 (2 2 9 8minvs 173 5min)延长 (P <0 0 5 ) ;与B组相比 ,R组Bromage评分最高分 (1 9vs 2 7)较低 ,起效时间 (14 4minvs2 5 0min)、Bromage评分≥ 1的维持时间 (173 5minvs 2 2 3 7min)较短 (P <0 0 5 ) ;RCLO组与B组的各指标均无显著性差异 ;BCLO组痛觉阻滞时间维持长于其它三组 ;R组Bromage评分≥ 1的维持时间明显短于其它三组。结论　可乐定辅助局
