抗血小板药物在冠心病患者二级预防中的应用现状调查

摘要
目的了解国内冠心病患者抗血小板药物的使用现状,并分析与用药相关的影响因素。方法借助一项国际多中心大规模临床试验在中国的实施,分别于2007年及2009年在14个城市的51家医院进行调查,对符合条件的冠心病患者,利用电子问卷采集临床资料,对经临床试验证实明确推荐用于二级预防的抗血小板药物使用情况进行统计描述和分析。结果调查的13078例冠心病患者中87.6%服用抗血小板药物,心肌梗死患者服药率为89.2%。既往接受冠状动脉血运重建术患者的抗血小板药物比例较高(94.0%)。各种抗血小板药物中,阿司匹林使用最为广泛,氯吡格雷次之,服药率分别为86.1%和24.6%。冠心病患者行经皮冠状动脉成形术治疗1年内联合使用阿司匹林及氯吡格雷的治疗率为81.0%(2058/2543),其中2009年较2007年有显著增加(86.0%vs.75.9%,P<0.001)。结论本项研究的结果显示国内冠心病患者抗血小板药物的使用与欧美国家使用情况相当,然而在女性及老年人群中抗血小板药物的使用率尚有待改善。
