老年卒中相关性肺炎患者的危险因素分析

摘要：

目的对老年卒中相关性肺炎的危险因素进行分析,探讨其临床干预措施。方法采用病例对照的研究方法,纳入住院的老年卒中相关性肺炎组患者178例,卒中未合并肺炎组患者150例,对两组患者的一般资料(年龄、性别、既往病史、吸烟史、营养状况),卒中类型、严重程度(意识水平、吞咽障碍、肢体反应)、生化指标等进行单因素分析,相关结果再采用多因素Logistic回归分析卒中相关性肺炎的危险因素。结果多因素Logistic回归分析显示年龄(OR=2.125),美国国立卫生院脑卒中量表(NIHSS)评分(OR=1.865),格拉斯哥昏迷评分(Glasgow Coma Scale,GCS)(OR=1.416),吞咽困难(OR=1.686),完全卧床(OR=2.008),合并高血糖(OR=1.123),低蛋白血症(OR=1.452),高皮质醇水平(OR=1.033),是老年卒中相关性肺炎发病的危险因素。结论老年卒中相关性肺炎的发生、发展存在多种高危因素,需要早期发现和预防。
