全喉切除术及颈淋巴结清除术对喉癌治疗的评价

摘要：

本文总结了中国医学科学院日坛医院自1958～1976年6月为止,采用全喉切除术（其中34例加用颈淋巴结清除术）治疗102例喉癌的临床经验。三年生存率为60.8%（62/102）,五年生存率为51.3%（40/78）,十年生存率49.0%（25/51）。文中对治疗五年以上的78例进行重点讨论。按手术方式统计五年生存率,全喉切除术为54.5%（24/44）,全喉手术加一侧颈淋巴结清除术为53.6%（15/28）,全喉加双侧颈淋巴结清除术为16.7%（1/6）。喉癌T3及T4病变为全喉切除术的适应证,本组70例T3、T4病变,治疗后五年内局部复发及颈部转移各为18.5%（各13例）。要有效的控制局部复发及减少颈部转移,就应适当考虑术前放疗及开展选择性颈淋巴结清除术。
