中老年椎体骨赘和腰椎间盘退变的相关性

摘要
目的:分析中老年椎体骨赘与腰椎间盘退变的相关性及探讨骨赘形成的可能机制。方法:回顾性分析120例中老年腰腿痛患者,利用X线及MRI分别观察骨赘和腰椎间盘退变,根据Nathan方法将骨赘分为4度,定义Ⅱ度及以上为存在骨赘,再根据Pfirrmann方法将腰椎间盘退变分为5级,对所测参数进行统计学分析。结果:骨赘与年龄在各腰椎节段均呈正相关,相关系数r差异具有统计学意义(P<0.05);下腰椎L3/4、L4/5的骨赘比例(76.7%、70.8%)比上腰椎L1/2、L2/3(31.7%、46.7%)高,骨赘形成和腰椎间盘退变在各腰椎节段均呈正相关,相关系数r具有统计学差异(P<0.01)。结论:随着腰椎间盘退变加重,椎体骨赘形成越明显,腰椎间盘退变导致椎体应力过高是骨赘形成重要原因。
