镜像治疗结合任务导向性训练对脑卒中患者上肢功能的影响

摘要
目的探讨镜像治疗结合任务导向性训练对脑卒中早期患者上肢功能的影响及可能机制。方法将21例脑卒中患者分为观察组(10例)和对照组(11例)。观察组患者在常规康复治疗基础上进行为期4周的镜像治疗结合任务导向性训练,对照组患者在与观察组相同的常规康复治疗基础上进行为期4周的上肢任务导向性训练,2组进行上肢训练的时间均为每天1 h,每周训练5 d。在治疗前及治疗2周后和4周后采用Fugl-Meyer运动功能评分法(FMA)(上肢部分)、偏瘫上肢功能测试(香港版)(FTHUE-HK)及Barthel指数(BI)对2组患者进行功能评估。结果在治疗2周后及4周后的评估中,2组患者的FMA(上肢部分)、FTHUE-HK及BI评分均较治疗前有显著提高(P<0.05)。在治疗2周后的评分中观察组仅FMA(上肢部分)提高的分值高于对照组(P<0.05);在治疗4周后的评估中FMA(上肢部分)及FTHUE-HK所提高的分值均高于对照组(P<0.05);而2次评估中BI评分的提高分值2组相比差异均无统计学意义(P>0.05)。结论镜像治疗结合任务导向性训练是一种有效的神经康复治疗方法,可用于改善脑卒中早期患者短期内的上肢功能,但是对于日常生活活动能力的改善尚不确定。
