血浆可溶性血栓调节蛋白与支架术后冠状动脉内皮化的相关性

摘要：

目的:探讨血浆可溶性血栓调节蛋白(soluble thrombmodulin,sTM)与支架术后冠状动脉(冠脉)内皮化的相关性,为临床ADP受体拮抗剂的用药时间提供参考。方法:分析2013-06-2014-08济宁医学院附属医院心内科收治的32例冠脉支架植入术后12～18个月无支架内再狭窄的冠心病患者的临床资料,对以上32例患者的34支血管35个支架进行光学相干断层成像(OCT)检查,根据OCT显示的支架小梁内皮化的程度将以上患者分为完全内皮化组和非完全内皮化组,同时选择18例正常成人作为对照组。采用ELISA的方法测定以上各组人群外周血sTM水平,并分析sTM的改变与支架术后冠脉内皮化之间的相关性。结果:非完全内皮化组、完全内皮化组和对照组血浆内sTM的水平分别为(7.98±1.43)μg/L、(5.25±1.62)μg/L和(5.21±1.55)μg/L,非完全内皮化组血浆内sTM的水平明显高于完全内皮化组和对照组(P<0.05),完全内皮化组血浆sTM因子水平与对照组之间无显著性差异(P>0.05),血浆sTM水平与支架小梁暴露数目呈正相关(r=0.723,P<0.05)。结论:血浆sTM与冠脉支架植入术后支架小梁暴露数目呈正相关,可为临床ADP受体拮抗剂的使用提供参考。
