摘要：

目的探讨急性冠状动脉(冠脉)综合征(acute coronary syndrome,ACS)并慢性肾功能不全行经皮冠脉介入(PCI)治疗的可行性。方法对我院收治的ACS并慢性肾功能不全行PCI治疗的1例的临床资料进行回顾性分析。结果本例因反复间断胸闷、呼吸困难、不能平卧半个月,加重并剧烈胸痛5 h入院。有高血压病、肾动脉狭窄、糖尿病、冠心病病史。2年前因肾功能进行性恶化,在外院行肾血管造影示:左肾动脉完全闭塞,右肾动脉明显狭窄,行右肾动脉支架置入术+左肾动脉球囊扩张术。此次入院诊断为ACS(多支病变),因肾功能不全不适合溶栓治疗,在进行充分的围术期准备后行经皮冠脉球囊成形术(PICA)及冠脉支架植入术(STENT),于冠脉前降支放置支架3枚。术后1周肾功能平稳,出院。结论采取选择合适的对比剂、减少对比剂用量和介入治疗时间、术前水化等措施,ACS并慢性肾功能不全患者可耐受介入手术,且术后效果较好。
