超广谱β-内酰胺酶分子生物学基础的研究进展

摘要
革兰氏阴性杆菌产超广谱 β-内酰胺酶 (ESBL s)是导致临床致病菌对新型广谱抗生素产生耐药性的重要机制之一。采用系统进化树的分析方法 ,对 ESBL s的氨基酸残基进行排序比较可进一步细分为 5个以上的亚群 ,同一亚群 ESBL s的功能特征具有较大的相似性。 ESBL s的产生机制可能涉及两个方面 :其一为基因转移 ,ESBL s的编码基因主要位于质粒 DNA序列中 ,由耐药基因和调控序列共同组成 ,此抗性基因盒可以通过转座子 (Tn)或整合子 (Int)及某些未知的转移机制在质粒间、质粒与染色体间乃至同种或不同种属的革兰氏阴性菌间迁移扩散 ;其二为基因变异 ,某些窄谱 β-内酰胺酶可以通过侧翼调控序列或耐药结构基因的突变 ,使其产酶效能得到大幅提高或使底物轮廓范围扩大 ,从而衍化为对多种氧亚胺基 β-内酰胺类抗生素具有催化水解效能的超广谱β-内酰胺酶
