重性抑郁障碍患者相关脑代谢物的水平变化及其作用研究进展

重性抑郁障碍(MDD)是常见的精神障碍,有高致残、致死率。MDD患者多个脑区存在不同程度的代谢物异常,脑代谢物水平的降低程度与细胞凋亡程度呈正比,且与患者过去的疾病负担有关。N-乙酰天冬门氨酸(NAA)是一种由线粒体产生的相对不活泼的氨基酸,可作为神经元活力、功能和神经纤维结构、功能完整性的标志,其水平还可提示神经元密度计功能状况。胆碱复合物可反映脑内的内总胆碱量,参与细胞膜的合成和降解,是膜磷脂代谢状态潜在的生物学标志,其水平代表细胞膜代谢。γ-氨基丁酸作为中枢神经系统中主要的抑制性神经递质,在MDD的发病机制中的作用越来越受到人们的重视。谷氨酸/谷氨酰胺复合物(Glx)包括谷氨酸、谷氨酰胺,二者波峰明显重叠,常作为一个整体被描述,当Glx水平高于NAA的1/3以上,提示Clx增高。肌酸是能量代谢的产物,是神经元组成成分,在维持细胞能量储存中发挥重要作用。肌醇参与磷酸肌糖循环,调节神经元渗透压、营养细胞、抗氧化剂生成表面活性物质,是神经胶质细胞的标志,也可能是神经毒性的指标。乳酸是糖酵解的代谢产物,正常脑组织中水平极低。
