老年急性ST段抬高心肌梗死患者血清白细胞介素6水平变化与预后的相关性

摘要：

目的探讨老年急性ST段抬高心肌梗死(acute ST-segment elevation myocardial infarction,STEMI)患者血清白细胞介素6(IL-6)水平变化与预后的相关性。方法选择发病6h内的老年STEMI患者147例作为病例组,无冠状动脉疾病的体检者143例作为对照组,采用ELISA法检测入选者血清IL-6水平。对病例组患者随访3年,住院期间死亡15例,随访3年死亡29例;记录基线及3年随访期的临床资料。结果病例组患者血清IL-6水平明显高于对照组(P<0.01)。住院期间及3年随访期死亡患者入院血清IL-6水平均明显高于存活患者(P=0.011,P=0.004)。以血清IL-6水平中位数5.97ng/L为界值,绘制Kaplan-Meier生存曲线,并进行log-rank检验显示,IL-6≥5.97ng/L患者3年生存率下降幅度明显高于IL-6<5.97ng/L患者(χ2=6.817,P=0.009)。结论老年STEMI患者血清IL-6水平升高,且其与近期及远期预后相关。
