新生儿脑白质损伤的病理特点和免疫学机制

摘要
新生儿脑白质损伤是一组具有多种病理类型的疾病 ,其病理特点主要包括反应性星形胶质化、髓鞘损害和轴突病变等 ,这些病变被认为是脑瘫发生的最重要的危险因素。新生儿脑白质损伤的发病机制也很复杂 ,近来研究发现免疫学机制在脑白质损伤过程中占重要地位。缺氧缺血或宫内感染等可激活受损局部的星形细胞和小胶质细胞 ,它们产生多种细胞因子 ,并和由血液中渗入的白细胞一起对这些细胞因子产生应答 ,进而损害脑白质组织中的星形细胞、少突胶质细胞以及轴突等 ,最终导致脑白质组织的坏死、星形胶质化、细胞凋亡以及囊腔形成等多种病理改变
