单髁置换术与全膝关节置换术后人工关节遗忘程度的比较

摘要
目的探讨分别接受膝关节单髁置换术(unicompartmental knee arthroplasty,UKA)和全膝关节置换术(total knee arthroplasty,TKA)治疗膝内侧骨关节炎患者术后主观感受上的差别,并通过人工关节被遗忘指数(forgotten joint scores,FJS)评分系统分析二者的差异性。方法回顾性分析2014年1月至2017年1月在我院同期行单侧膝关节UKA与TKA治疗膝骨关节炎的患者共60例,分为UKA组和TKA组,每组30例患者,其中UKA组男18例,女12例;年龄55~76岁,平均(63.9±6.7)岁;身体质量指数(body mass index,BMI)平均为(22.4±1.5)kg/m2。TKA组男17例,女13例;年龄55~75岁,平均(62.6±5.6)岁;BMI平均为(22.0±1.1)kg/m2。分别记录两组患者术后1年和末次随访时的FJS评分和美国特种外科医院(the hospital special surgery,HSS)膝关节评分。结果所有患者均获随访,随访时间12~38个月,平均(18.3±3.9)个月。术后1年和末次随访时UKA组FJS评分分别为(70.6±3.3)分、(71.8±3.2)分;TKA组术后1年及末次随访时FJS评分分别为(59.3±3.1)分、(59.8±2.4)分;两组患者在同一随访时间段内组间FJS评分比较差异有统计学意义(P<0.05),而同组间FJS评分在同一随访时间段比较差异无统计学意义(P>0.05)。术后随访1年时UKA组的HSS评分明显优于TKA组(P<0.05),末次随访时,两组患者的HSS评分比较差异无统计学意(P>0.05)。结论在治疗膝关节内侧间室骨关节炎方面,接受UKA的患者比接受TKA的患者假体的自我化程度更高,初期疗效也更加明显。
