脑微出血与慢性脑缺血患者的认知损害

摘要
目的探讨脑微出血(cerebral microbleeds, CMBs)与慢性脑缺血患者认知损害的相关性。方法纳入2015年1月至2017年1月在河北医科大学第二医院神经内科住院的慢性脑缺血患者,依据磁敏感加权成像结果分为CMBs阳性组和阴性组,采用简易精神状态检查表(Mini-Mental State Examination, MMSE)及蒙特利尔认知评估量表(Montreal Cognitive Assessment, MoCA)比较2组患者的认知功能,并分析不同部位CMBs对认知功能的影响。依据MoCA评分分为认知功能损害组和无认知功能损害组,采用多变量logistic回归分析确定认知损害的独立危险因素。结果共纳入96例患者,年龄45~83岁,男性64例(66. 7%),女性32例(33.3%);CMBs阳性组51例(53.1%),阴性组45例(46. 9%);认知损害组33例(34. 4%),无认知损害组63例(65. 6%)。在CMBs阳性组中,单纯脑叶CMBs 10例,脑深部及幕下CMBs 29例,混合型CMBs 12例。多变量logistic回归分析显示,年龄[优势比(odds ratio, OR) 1. 115,95%可信区间(confidence interval, CI) 1.030~1.207;P=0.007]、CMBs数量(OR 1.254,95%CI 1.064~1.479;P=0.007)、Fazekas评分(OR 2.697,95%CI1. 012~7.185;P=0. 047)和多发腔隙性梗死(OR 7. 103,95%CI 1. 248~40. 424;P=0. 027)为认知损害的独立危险因素。与CMBs阴性组比较,CMBs阳性组认知损害及痴呆比例显著较高,MoCA总分以及视空间和执行功能、注意、延迟回忆3个认知域评分显著较低(P均<0. 05)。单纯脑叶CMBs组各认知域评分均显著低于非脑叶CMBs组(P均<0.05);单纯深部及幕下CMBs组各认知域评分与非深部及幕下CMBs组差异无统计学意义(P均>0. 05);混合部位CMBs组视空间与执行功能、注意力、延迟回忆3个认知域评分均显著低于非混合CMBs组(P均<0.05)。结论 CMBs(尤其是脑叶CMBs)会导致慢性脑缺血患者出现全面认知功能下降甚至痴呆,且与视空间与执行功能、注意、延迟回忆3个认知域功能关系最为密切。
