慢性心力衰竭患者血清脂联素的浓度及临床意义

摘要
目的测定慢性心力衰竭(CHF)患者血清脂联素(APN)浓度,探讨在中国人群中血清APN对CHF患者预后的预测作用。方法采集症状基本得到控制的CHF患者血清,测定APN与N端前脑钠素(NT-proBNP)、肿瘤坏死因子α(TNF-α)、白细胞介素6(IL-6)、高敏C反应蛋白(hsCRP)浓度,结合临床CHF严重程度,研究APN在预测CHF患者死亡率、再入院率的潜在价值。结果CHF患者APN水平显著升高。随CHF的严重程度(NYHA分级Ⅱ~Ⅳ)APN水平逐渐升高(P<0.05),APN与NT-proBNP正相关(r=0.48,P<0.001),与hsCRP负相关(r=-0.34,P<0.001)。血清APN水平升高伴随有死亡率的增加。结论中国CHF患者APN水平显著升高;APN可预测CHF的死亡率和复发率。
