老年再发脑梗死患者阿司匹林抵抗及相关因素分析

摘要：

目的探讨再发脑梗死老年患者中发生阿司匹林抵抗(AR)的概况及相关因素。方法 77例再发脑梗死老年住院患者,发病前连续服用阿司匹林3～38个月,每天100 mg。住院第2天晨起采集空腹静脉血,分别以二磷酸腺苷(ADP)、花生四烯酸(AA)为诱导剂,检测血小板聚集功能。结果用光学法测定血小板聚集功能,患者中AR发生率为23.4%(18例),阿司匹林半抵抗(ASR)发生率15.6%(12例);AR/ASR总发生率39.0%(30例);吸烟者较多(73.3%,P=0.001),并与高敏C反应蛋白(hsCRP)存在相关性(β=-1.643,χ2=10.534,P=0.003,OR=0.001,95%CI:-0.849～-0.242)。结论再发缺血性脑血管病老年患者中存在较高比例的AR及ASR,其发生与吸烟、炎症有一定相关性。有效预测AR/ASR及抗血小板治疗个体化,将使病人在二级预防的治疗中受益更大。
