原发性高血压患者升主动脉弹性与靶器官损害关系的初步研究

摘要
目的探讨高血压患者升主动脉弹性与靶器官损害(TOD)的关系及敏感指标。方法纳入诊断为原发性高血压的192例患者,按伴发TOD个数分为A组51例(不伴TOD),B组73例(伴发1个TOD),C组48例(伴发2个TOD),D组20例(伴发3个TOD)。回顾性分析心电门控冠状动脉CT成像检查。采用动脉可扩张度(AD)、动脉顺应性(AC)、动脉僵硬度(ASI)指标评价升主动脉弹性。结果 4组AD、AC、ASI、舒张期标化最小直径(Ds)、舒张期标化最小面积(Ss)和肱踝脉搏波传导速度(baPWV)比较,差异有统计学意义(P<0.05,P<0.01)。A组与B组AD[(3.1±1.2)10-3/mm Hg vs(1.7±0.8)10-3/mm Hg(1mm Hg=0.133kPa)]、AC[(2.2±0.6)mm2/mm Hg vs(1.4±0.7)mm2/mm Hg]、ASI(6.8±2.6 vs 13.7±7.2)和baPWV[(1404.0±123.0)cm/s vs(1638.0±307.0)cm/s]比较,有显著差异(P<0.01)。B组与C组AD和AC比较,差异显著(P<0.01)。C组与D组ASI比较,差异显著(P<0.01)。A组与B组,B组与C组标化Ds和Ss比较,均有显著差异(P<0.05)。中分位组患TOD风险是低分位组的7.2倍。高分位组患TOD风险是中分位组的9.8倍。结论冠状动脉CT检查升主动脉弹性指标中,AD较AC和ASI更能敏感地预测原发性高血压患者亚临床TOD,并优于传统baPWV和形态学。
