炎症因子与心率变异性在不稳定心绞痛中的相关性研究

摘要
目的研究不稳定心绞痛(UA)患者炎性标志物与心率变异性之间的关系。方法自2013年6月至2014年6月在山东中医药大学附属医院心内科住院的UA患者40例,女性26例,男性14例,年龄5080岁。将研究对象分为hs-CRP<1.0组mg/L(n=15),hs-CRP(1.03.0)mg/L组(n=16),hs-CRP>3.0 mg/L组(n=9)。所有患者进行24 h动态心电图检测获取心率变异性(HRV),计算时域指标及频域指标。同时检测炎症因子超敏C反应蛋白(hs-CRP)和白介素-6(IL-6)含量。分析hsCRP、IL-6的自然对数与SDNN、RMSSD的关系。结果 hs-CRP(>3 mg/L)组较hs-CRP(13 mg/L)组SDNN和RMSSD均降低,差异有统计学意义(P均<0.05)。各组间频域指标比较,随hs-CRP的升高,其低频(LF)、高频(HF)、极低频(VLF)和总功率(TP)逐渐降低,但各组间差异无统计学意义(P均>0.05)。单因素相关分析显示hs-CRP自然对数(Lnhs-CRP)与SDNN之间存在负相关性(r=-0.365,P=0.022),而Lnhs-CRP与RMSSD之间无明显相关。IL-6自然对数(Ln IL-6)与SDNN呈负相关(r=-0.700,P<0.001),与RMSSD呈负相关(r=-0.457,P=0.003)。结论 UA患者HRV的降低与体内炎性标记物hs-CRP、IL-6增加相关。
