DiscoverTM假体高度对颈椎人工椎间盘置换术置换节段生物力学影响的三维有限元分析

摘要
目的 观察DiscoverTM假体高度对颈椎人工椎间盘置换术(CTDR)置换节段生物力学的影响。方法 在全颈椎三维有限元模型基础上建立采用5、6、7 mm 3种不同高度DiscoverTM假体行C5/C6单节段CTDR的颈椎模型,比较不同高度假体对置换节段活动度(ROM)、假体与骨终板接触面的应力分布及邻近节段椎间盘应力的影响。结果 3种高度的假体在不同方向上均能较好地维持或增加置换节段ROM。假体与骨终板接触面在各个方向上的应力随着假体高度的增加而增加;在后伸时,与5 mm假体相比,6 mm假体与上、下骨终板接触面的平均应力分别增加了52%和260%,7 mm假体分别增加了242%和720%。3种高度的假体均在后伸位的上终板接触面应力最高,5 mm假体在后伸位与骨终板接触面的应力在上终板两侧最大、后侧最小;6 mm假体与骨终板接触面的应力变化与运动方向基本一致;7 mm假体在后伸位与骨终板接触面应力在上终板后侧最大。在前屈位上,邻近节段应力随假体高度的增大而减少,在后伸、左右侧曲及左右旋转时,相邻节段应力则随着假体高度的增大而增大。结论 CTDR能较好地维持置换节段ROM。在维持合适椎间高度前提下,需选取较低高度假体以减小假体与骨终板接触面的应力,从而降低术后异位骨化及邻近节段退行性变的发生率。
