丙泊酚序贯右美托咪定对ICU躁动患者脱机过程中镇静效应的影响

摘要
目的:探讨丙泊酚序贯右美托咪定对ICU躁动患者脱机过程中实施浅镇静的效应。方法:选择机械通气治疗躁动患者80例,随机分为两组。丙泊酚序贯右美托咪定组(A组)先微量持续泵入丙泊酚0.3~3 mg/(kg·h),脱机前24 h,改右美托咪定维持剂量0.2~1μg/(kg·h),若24 h未脱机成功,泵至脱机前停用。丙泊酚持续泵入组(C组):持续泵入丙泊酚0.3~3 mg/(kg·h),若24 h未脱机成功,泵至脱机前停用。所有患者监测至脱机后60 min,每4小时进行一次镇静评分,调整剂量使患者RASS评分控制在-2~1分。分别记录脱机前24 h(T1)、脱机前12 h(T2)、脱机拔管时(T3)、脱机后30 min(T4)、脱机后60 min(T5),药物对HR、MAP、拔管时间、不良反应发生情况、ICU留置时间及住院时间的影响。结果:两组均达到ICU患者所需镇静效果,A组较C组循环波动少、拔管所需时间、ICU置留时间及住院时间短、患者谵妄、恶心呕吐等不良反应少,差异有统计学意义(P<0.05)。结论 :丙泊酚序贯右美托咪定对ICU机械通气患者实施浅镇静,镇静效果满意,血流动力学稳定,无明显呼吸抑制,不良反应低。
