前列地尔注射液改善兔耳静脉瘀血皮瓣微循环的研究

摘要
目的:观察应用前列地尔注射液对兔耳静脉瘀血皮瓣微循环的影响。方法:选取健康普通大耳白兔16只,随机分成生理盐水对照组及前列地尔注射液治疗组,每组8只。麻醉后于左耳背制作大小6cm×3cm的静脉瘀血皮瓣模型。皮瓣成形术后立刻于对照组右耳缘静脉缓慢注入生理盐水2ml,治疗组缓慢注入前列地尔注射2ml(1μg/1ml),每天同一时间点注射一次至实验结束,观察皮瓣外观及皮瓣存活率,分别于术后1、3、5、7d,各组分别随机取1只,切取皮瓣中点处皮瓣组织,HE染色观察红细胞瘀积及炎症细胞浸润等情况,免疫组织化学染色观察皮瓣中VEGF及HIF-1α的表达情况。结果:治疗组皮瓣肿胀情况、瘀血情况均明显较对照组轻。治疗组皮瓣成活率为(90.80±3.27)%明显高于对照组的(67.20±2.59)%,差异有统计学意义(P<0.05)。HE染色结果示治疗组红细胞瘀积及炎症细胞浸润等情况明显较对照组轻;免疫组织化学结果示治疗组皮瓣中VEGF的阳性表达量明显高于对照组,HIF-1α的阳性表达量明显低于对照组。结论:前列地尔注射液通过促进皮瓣组织中VEGF的表达和降低皮瓣中HIF-1α的表达量,促进瘀血皮瓣中新生血管的生成速度及减轻瘀血皮瓣的缺血缺氧双重作用改善瘀血皮瓣的微循环,提高静脉瘀血皮瓣的存活面积。
