高屈曲旋转平台型假体全膝关节置换的近期随访:膝关节活动度及功能评价

摘要
背景:人工全膝关节置换技术在临床上已经成熟。LPS-Flex Mobile Bearing System(Zimmer,USA)人工膝假体为旋转平台高屈曲型膝关节假体,由于国内临床应用时间较短,该假体的临床应用优越性目前尚未见报道。目的:探讨应用LPS-Flex Mobile Bearing System人工膝假体(Zimmer,USA)行全膝关节置换的近期疗效,以置换后膝关节活动度及功能评价人工假体与宿主的生物相容性。方法:回顾性分析2012年2月至2014年3月苏州大学附属第一医院收治的以Zimmer LPS-Flex Mobile Bearing System人工膝假体(高屈曲旋转平台型膝关节假体)行全膝关节置换的患者37例(42膝),男9例(10膝),女28例(32膝);年龄47-78岁,平均63.7岁。采用骨水泥固定,不进行髌骨置换。观察置换后并发症,比较置换前、置换后膝关节活动度,并根据纽约特种外科医院膝关节评分评估膝关节功能恢复情况。结果与结论:共34例(38膝)获得随访,随访时间6-28个月。膝关节活动度从置换前平均88.5°改善到置换后平均124.2°;置换前纽约特种外科医院膝关节评分平均为52.5分,置换后平均为91.1分,均有显著提高(P<0.01)。根据纽约特种外科医院膝关节评分系统评定疗效:优20例,良16例,中2例,优良率达95%。各种并发症的发生率低。提示高屈曲旋转平台型假体人工全膝关节置换的近期临床效果满意,该假体在设计上更具优势,更加接近膝关节生理结构,但长期效果还需进一步观察。
