饮食和运动治疗对2型糖尿病患者血清抵抗素水平的影响及其与肥胖相关性分析

摘要
目的探讨饮食、运动治疗对2型DM患者血清抵抗素水平的影响及其与肥胖的关系。方法选取DM患者32例,随机分为试验组(饮食控制和运动治疗)及对照组。检测2组血清抵抗素、腰围、BMI、空腹胰岛素、糖化血红蛋白(HbA1c)。结果观察前试验组与对照组比较BMI、腰围、空腹胰岛素、HOMA-IR、HbA1c及血清抵抗素水平差异均无统计学意义(P>0.05),12周后各项指标与观察前比较差异有统计学意义(均P<0.05)。2组间比较,试验组各项指标改善均优于对照组,且差异有统计学意义(均P<0.05)。相关性分析显示:在试验组中,血清抵抗素与BMI(r=0.62,P=0.01)、腰围(r=0.81,P=0.03)、空腹胰岛素(r=0.68,P=0.01)、HOMA-IR(r=0.70,P=0.03)、HbA1c(r=0.81,P=0.03)均呈正相关。在对照组中,血清抵抗素与BMI未显示相关性(P>0.05),而与腰围(r=0.61,P=0.01)、空腹胰岛素(r=0.47,P=0.02)、HOMA-IR(r=0.61,P=0.01)、HbA1c(r=0.74,P=0.03)均呈正相关。结论饮食、运动治疗可降低2型DM患者血清抵抗素水平,血清抵抗素与腰围、空腹胰岛素、HOMA-IR及HbA1c关系密切。
