老年患者脑卒中后失语症应用Loewenstein认知评定量表的临床研究

摘要：

目的探讨Loewenstein认知评定量表(LOTCA)在老年脑卒中后失语症中的临床应用价值。方法选择老年脑卒中后失语症患者22例,应用LOTCA测评患者认知功能,并采用西方失语症成套量表(WAB)和标记测验评定其LOTCA测评前后的语言功能。对LOTCA与WAB及标记测验之间进行相关性分析,对LOTCA测评前后的语言功能进行对比分析。结果 22例患者LOTCA总分与标记测验成绩及WAB中各个亚项、失语商、操作商、大脑皮质商呈正相关(P<0.05,P<0.01);WAB中失语商、标记测验与LOTCA中定向、空间知觉、动作运用及总分呈正相关(P<0.05,P<0.01);多元逐步回归分析显示,影响患者失语商的主要因素为定向、动作运用、视知觉。与测评前比较,测评后患者的自发言语、听理解、复述、命名、书写、失语商评分明显升高,差异有统计学意义(P<0.05)。结论 LOTCA是一套适合于老年脑卒中后失语症患者行之有效的认知测评量表。
