急性脑梗死患者血抵抗素、脂联素水平的改变及其与颈动脉粥样硬化和脑卒中危险因素的关系

摘要
目的探讨急性脑梗死患者血抵抗素、脂联素水平的改变及其与颈动脉粥样硬化和脑卒中危险因素的关系。方法检测113例急性脑梗死患者的血清抵抗素、脂联素(酶联免疫吸附测定法)及血糖、血脂、超敏C反应蛋白(hs-CRP)水平和血压,用彩色多普勒超声仪检测颈动脉;并与正常对照组进行比较。对血抵抗素水平与其他观察指标的相关性及影响因素进行分析。结果急性脑梗死组的血清抵抗素水平显著高于正常对照组(P<0.001)、脂联素水平显著低于正常对照组(P<0.01)。多因素非条件Logistic回归分析显示,抵抗素、年龄、血压、低密度脂蛋白(LDL)、hs-CRP为颈动脉粥样硬化的危险因素。血抵抗素水平与血糖、LDL、hs-CRP、颈动脉内-中膜厚度(IMT)、粥样硬化斑块评分呈正相关(r=0.16~0.31,均P<0.05),与脂联素、高密度脂蛋白(HDL)呈负相关(r=-0.23~-0.26,均P<0.05)。结论急性脑梗死患者的血清抵抗素水平显著升高、脂联素明显降低。抵抗素是颈动脉粥样硬化的危险因素,与血糖、LDL、hs-CRP和颈动脉粥样硬化程度正相关;而脂联素与其呈负相关。
