本体感觉功能与慢性踝关节不稳相关性的初步研究

摘要
目的观察正常人和慢性踝关节不稳(CAI)患者踝关节位置觉和腓骨肌的反应时间, 比较正常踝关节和受损关节间差异, 评估本体感觉对踝关节功能的影响。方法选取CAI患者21例(实验组)、踝关节正常人30例(对照组), 在非负重静息状态、角速度2°/s条件下, 利用踝关节被动位置觉角度重现法测定2组研究对象踝关节位置觉感应能力, 并在佩戴与不佩戴护踝两种条件下模拟踝关节突然内翻, 测定腓骨肌反应时间。结果与对照组比较, 实验组在设定内翻目标角度为20°、30°的条件下, 其踝关节本体感觉位置觉角度差值均较对照组大, 差异有统计学意义(P<0.05)。与对照组同等条件下比较, 实验组在佩戴和不佩戴绷带式护踝条件下, 腓骨长肌(PL)、腓骨短肌(PB)的肌肉反应时间(MRT)均较对照组高, 差异有统计学意义(P<0.05)。2组受试者组内比较, 佩戴与不佩戴绷带式护踝条件下PL、PB的MRT差异均无统计学意义(P>0.05)。结论慢性踝关节外侧不稳定患者位置觉感应能力下降, 腓骨肌反应时间延长, 护踝对于受损踝关节具有良好的机械性保护作用, 但对于MRT没有显著提升作用。
