慢性阻塞性肺病肌肉功能与肺功能的相关性

摘要
目的:探讨慢性阻塞性肺病患者肌肉功能与肺功能的相关性。方法:87例慢性阻塞性肺病(chronic obstructive pulmonary disease,COPD)患者进行肘屈伸、膝屈伸的等速肌力测试,记录60°/s和180°/s角速度时每次屈伸运动的峰力矩。结果:肘、膝屈伸力量与运动后心率、呼吸频率和6min步行试验呈正相关,与运动后血氧饱和度呈负相关,伸膝疲劳性与6min步行试验、运动心率、运动后呼吸频率呈负相关,与运动后血氧饱和度呈正相关。肘、膝屈伸力量与舒张后一秒用力呼气容积(forced expiratory volume in one second,FEV1)(r=0.297—0.369,P<0.05)、最大呼气流量(peak expiratory flow,PEF)舒张后(r=0.19,P>0.05;r=0.254—0.313,P<0.05)、深吸气量(inspiratory capacity,IC)舒张后(r=0253—0.312,P<0.05)、6min步行测试后IC(r=0.288—0.342,P<0.05)呈正相关。结论:COPD患者肌肉功能减弱可能是全身性的,与过度充气存在一定关系。
