应激性高血糖对脑梗死的影响

摘要：

应激性高血糖可使脑梗死溶栓治疗后有症状出血率增高,影响溶栓治疗的效果。应激性高血糖通过加重脑组织内乳酸堆积,增加自由基形成和兴奋性氨基酸的产生等多种途径加重缺血性脑损伤。其作用受梗死范围、部位和局部血液循环情况等多种因素的影响。目前是否应用胰岛素降低应激性高血糖,降低到什么程度合适尚无统一标准,建议在脑梗死患者入院后24 h内避免静脉输注葡萄糖溶液。
