促炎症和抗炎症细胞因子基因表达与脓毒症小鼠肝损伤的关系

摘要
目的 探索促炎症及抗炎症细胞因子在脓毒症小鼠肝损伤中的作用。方法 盲肠结扎穿孔 (CLP)造成小鼠脓毒症模型 ,假手术 (sham)组接受同样手术操作但不行CLP。用RT PCR的方法检测了脓毒症小鼠肝组织中多种细胞因子 (促炎症细胞因子TNFα、IL 1β、IL 6 ,抗炎症细胞因子IL 4)mRNA的表达 ,同时还测定了肝组织含水量及微血管通透性的改变。结果 与sham组相比 ,CLP小鼠在 3h后TNFα、IL 1β基因表达即明显增高 (P <0 0 1) ,至 12h仍然高于sham组 (P <0 0 1) ,但较 3h有所降低 ;IL 6基因在CLP后 3h也明显增高 (P <0 0 1) ,并且在CLP后 12h仍继续增高 (P <0 0 1) ;与之不同的是IL 4基因 ,在CLP后 3hIL 4基因表达与sham组差异无显著意义 ,至CLP后 12h才明显高于sham组 (P <0 0 1)。CLP后 3h肝组织含水量与肝微血管通透性即开始有所升高但与sham组比较差异无显著意义 ,至CLP后 12h进一步增高与sham组比较差异均有显著意义 (含水量P <0 0 5 ,微血管通透性P <0 0 1)。结论 促炎症细胞因子与抗炎症细胞因子之间力量对比的失衡可能是脓毒症并发肝损伤的重要原因
