社区获得性肺炎院外经验性治疗的疗效与费用分析

摘要：

目的 :探讨国人社区获得性肺炎 (CAP)的院外治疗是否安全有效及其疗效费用分析。方法 :本研究前瞻地研究 10 0例无合并症和并发症的CAP ,患者根据病情轻重分别给予静脉或口服抗生素治疗。青霉素V组 2 6例 ,罗红霉素组 2 7例 ,头孢呋辛组 2 6例 ,青霉素G组 2 1例 ,失访 3例。结果 :4组单一用药治疗成功病例分别为 2 0例(83% ) ,2 6例 (10 0 % ) ,2 6例 (89% )和 2 1例 (10 0 % ) ,仅 1例患者因肺功能衰竭住院接受机械通气治疗 ,其余患者均在急诊室随诊治疗 ,各组的疗效无显著性差别。口服给药组的抗生素费用及成本费用均显著低于静脉给药组。结论 :对于年轻无合并症的CAP患者口服抗生素院外治疗是有效、安全、经济的治疗方案 ,同静脉注射抗生素相比可大大节省医疗费用 ,大环内酯类可能是首选药物。
