冠状动脉钙化斑块对CTA诊断管腔狭窄的影响及临床意义

摘要：

目的:通过点对点与冠状动脉造影检查(CAG)比较,评价CTA观察钙化斑块对于局部管腔狭窄影响的特点和一般规律,从而提高CTA诊断钙化斑块所致管腔狭窄的效能。方法:回顾性分析我院接受64排螺旋CT冠状动脉成像和CAG的41位患者的影像资料。以CAG为参考标准,通过CTA对钙化斑块形状、分布及钙化斑块的长、宽径、体积、钙化积分及局部管腔狭窄与CAG点对点测定对应冠状动脉的管腔狭窄程度。结果:CTA显示的钙化斑块局部管腔狭窄程度(52%±13.1%)明显大于CAG(21%±22.6%)(P<0.05)。CTA诊断钙化斑块局部管腔显著性狭窄的敏感度、特异度、阳性预测值、阴性预测值、准确性分别为81.2%,40.3%,37.8%,92.4%,51.5%。结论:钙化斑块的长径、宽径及体积越大,钙化积分越高,局部管腔显著性狭窄的几率越高。CTA诊断钙化斑块局部管腔狭窄程度的敏感度和阴性预测值较高,特异度、阳性预测值和准确性较低。
