重型颅脑损伤患者颅内压监测及护理体会

摘要：
目的探讨重型颅脑损伤患者进行颅内压监测的护理经验。方法应用Codman颅内压监护仪对45例重型颅脑损伤患者进行监测及护理:重视基础护理,加强并发症的防治,准确测量颅内压并根据不同情况及时做出相应处理。结果45例患者经过持续颅内压监测,10例患者病情发生变化,颅内压超过5.3kPa,急诊手术去除骨瓣减压,9例术后颅内压下降,恢复良好;1例术后再次升高,患者家属拒绝再次手术,最终死亡。余35例患者于病程的2～6d颅内压出现增高,但未达手术要求值,经过脱水等处理,颅内压下降。根据格拉期哥预后量表(GOS)评分进行划分,本组存活44例,其中Ⅰ级22例,Ⅱ级11例,Ⅲ级11例;Ⅴ级:死亡1例。结论动态监测颅内压的变化有助于准确判断病情,辅以细致周到、科学合理的护理措施,对改善重型颅脑损伤患者的预后有重要意义。
