冠心病支架植入术后消化道出血的临床分析

摘要：

目的评价冠脉介入术后患者长期双联抗血小板药物治疗相关消化道出血的发生率、病因及转归。方法回顾性分析冠心病药物支架置入术后接受双联抗血小板治疗患者1825例,观察所有患者的临床指标,确定消化道出血的发生率、病因及临床后果。结果 28例患者发生严重消化道出血,累积发生率为1.53%,上消化道出血占64.3%,消化性溃疡占39,3%。与对照组相比,严重出血组年龄偏大(P<0.05),多见于应用血小板Ⅱb/Ⅲa受体拮抗剂(P<0.01)、既往有长期口服阿司匹林(P<0.01)、消化道出血史(P<0.01)、消化性溃疡史者(JP<0.01)。Kaplan—Meier分析:冠脉介入术后患者消化道出血与严重心血管事件相关并有统计学意义(P<0.01)。结论冠脉介入术后随访消化道出血患者严重心血管不良事件发生率显著增高。
