股骨近端髓内钉与动力髋螺钉治疗老年股骨转子间及转子下骨折的比较研究

摘要：

目的对采用股骨近端髓内钉(proximalfemoralnail,PFN)及动力髋螺钉(dynamichipscrew,DHS)治疗老年股骨转子间及转子下骨折的操作情况、侵袭性、术后骨折愈合时间、并发症及功能恢复等情况进行比较。方法2002年1月～2003年6月,随机将股骨转子间及转子下骨折的患者分为两组,一组采用DHS治疗,即DHS组,另一组采用PFN治疗,即PFN组。分别对两组患者的一般资料,麻醉评分,手术资料及术后功能恢复情况进行评估比较。结果平均手术时间DHS组62.8min,PFN组45.4min;术中出血量DHS组230ml,PFN组164ml;平均切口长度DHS组13.7cm,PFN组8.7cm;两组比较差异有显著性。术后引流量DHS组120ml,PFN组106ml,两组比较差异无显著性。DHS组89例,84例获得随访,随访时间12～25个月,平均18.6个月;PFN组78例,74例获得随访,随访时间12～21个月,平均18.2个月。DHS组平均愈合时间为4.6个月,有2例深部血肿,6例复位不满意;PFN组平均愈合时间为4.3个月,有1例深部血肿,5例不满意,两组比较差异无显著性意义。两组患者均于术后12个月测量双下肢长度:DHS组患侧较健侧平均缩短1.4cm;PFN组患侧较健侧平均缩短0.8cm,两组比较差异无显著性意义。结论应用DHS和PFN治疗老年股骨转子间及转子下骨折时,在骨折愈合时间、术后并发症及术后功能方面无明显差异
