颅脑手术后颅内感染相关危险因素分析与防治对策

摘要
①目的探讨开颅手术后颅内感染的相关危险因素及其防治措施。②方法对10年来不同颅脑手术患者3932例的临床资料及对发生颅内感染患者的危险因素进行回顾分析。③结果术后发生颅内感染者136例,感染率为3.46%;多种不同的危险因素与颅内感染的发生有关,如术后有脑脊液漏、切口皮下反复积液、长时间脑室引流及急症术后再次手术者,其术后颅内感染的发生率分别为42.20%、31.87%、26.67%和21.43%,明显高于平均感染率(P<0.01);有颅脑手术史者术后颅内感染发生率为5.65%,幕下手术者术后颅内感染发生率为6.15%,手术时间>8小时者术后颅内感染发生率为7.87%,术前有糖尿病者术后发颅内感染生率为11.72%,术前有肺部感染者术后发颅内感染生率为9.52%。④结论术后有脑脊液漏、切口皮下反复积液、长时间脑室引流及急症术后再次手术是术后发生颅内感染的高危因素。严格的无菌操作、围术期使用有效抗生素、针对不同的危险因素采取相应的预防措施,可减少术后颅内感染的发生。
