颈性眩晕的发病机制、诊断和治疗研究进展

摘要：

颈性眩晕以源于颈椎的眩晕和不平衡为特点,其发病机制尚不完全清楚,目前主要有6种学说分别从不同角度探讨了颈性眩晕的发病机制,包括本体感受器紊乱学说、交感神经功能刺激学说、椎基底动脉供血不足学说、与偏头痛及颈源性头痛相关的颈性眩晕学说、与颈椎失稳相关的颈性眩晕学说及体液因子学说。由于颈性眩晕患者的主诉多而客观体征少,其诊断要在排除其他可能疾病的基础上,通过进一步的相关体格检查及影像学检查来确诊。手法、封闭疗法、物理疗法及药物疗法是临床常用的治疗颈性眩晕的非手术疗法;对于诊断明确,反复发作,严重影响日常生活和工作,且非手术治疗无效的患者,目前常采用颈椎前路或后路减压、椎间植骨融合术,通过进行椎动脉减压和恢复颈椎的稳定性进行治疗。继续努力探索颈性眩晕的发病机制,制定诊断和治疗的专家共识,是目前的当务之急。
