年轻乳腺癌患者诊疗进展

摘要：

目的:总结年轻乳腺癌患者的临床病理特征、早期诊断方法、综合治疗获益及化疗后保留生育功能的研究进展。方法:以"年轻乳腺癌,特征,诊断和治疗"为关键词,检索Medline及中文全文期刊数据库1990-01-2008-12的相关文献,获得英文文献2664篇,中文文献121篇。纳入标准:1)年轻乳腺癌患者临床及病理特征;2)年轻乳腺癌患者诊断方法;3)年轻乳腺癌患者综合治疗获益;4)年轻乳腺癌患者化疗后闭经及生育功能的保留。根据纳入标准精选72篇文献,最后纳入分析42篇文献。结果:年轻乳腺癌患者较年长者具有更多不良预后特征。随着综合治疗手段的进步,年轻的乳腺癌患者有了更多综合治疗的选择,但疗效尚未确定。对于化疗后闭经的年轻患者,其生育功能的保留从技术上是可行的。结论:年轻乳腺癌患者具有不良的预后特征,相对年长乳腺癌患者预后较差。针对年轻乳腺癌患者的循证医学证据尚不充分,其定义、诊疗规范还有待于进一步循证以达成共识。
