国外局部外用药控制膝骨关节炎疼痛的临床研究进展

摘要：

膝关节骨性关节炎(knee osteoarthritis,KOA)疼痛非常复杂,口服药物不良反应多,不利长期应用。本文介绍局部外用药用以控制KOA源性疼痛的现状。检索Pub Med、EMBASE、Cochrane临床对照试验库、Cochrane图书馆,检索时间均从1990年1月至2014年6月。根据局部外用药物用以治疗KOA的临床试验相关报道,共纳入16种药物,其中欧盟流通非甾体抗炎药物7种:酮洛芬、双氯芬酸、氟比洛芬酯、吡罗昔康透皮贴剂、布洛芬、吡罗昔康和尼氟灭酸凝胶;酰胺类局麻药物1种:利多卡因;阿片类药物2种:芬太尼和曲马多;草本药提取物4种:辣椒素、山金车、橄榄油和紫草科植物。外治法药物控制KOA疼痛,首推双氯芬酸、酮洛芬、吡罗昔康,起效快、安全性高。草本类药物橄榄油、山金车、辣椒素、紫草科植物萃取物可供选择。局部药物外治是KOA镇痛的重要选择,国外此方面研究方兴未艾,国内需及时跟进。
