青年冠心病临床特征及预后分析

摘要：

目的:探讨青年冠心病患者的临床特征及预后情况。方法:将573例冠心病患者以40岁和60岁为界分为青年组(年龄≤40岁,116例)和老年组(年龄≥60岁,457例),对其临床特征、治疗及预后情况进行回顾性分析。结果:青年组116例中男性患者、体重指数、吸烟及长期饮酒比列均显著高于老年组(P<0.05),而有高血压及2型糖尿病病史者则较少(P<0.01),有冠心病家族史者则更多(P<0.01)。冠脉造影比较方面,与老年组相比,青年组中单支病变更多(P<0.01),三支病变较少(P<0.01);但在病变累及具体分支的比较上,青年组与老年组间无显著性差异(P>0.05)。预后方面,青年组1、2、3年的主要心脏不良事件(MACE)发生率和病死率均显著低于老年组(P<0.05)。结论:男性、肥胖、吸烟、长期饮酒、冠心病家族史和高TG血症是青年冠心病的独特重要的危险因素;青年冠心病的病变范围及程度要明显轻于老年患者,MACE发生率和病死率也显著低于老年患者。
