糖尿病与食管癌病例对照研究的Meta分析

摘要：

目的探讨糖尿病与食管癌的关系。方法以"食管癌、食管肿瘤、糖尿病、esophageal cancer、esophageal neoplasms、diabetes mellitus"为关键词,计算机检索中国期刊全文数据库(CNKI)、中国生物医学文献数据库(CBM)、维普中文科技期刊数据库(VIP)、Cochrane Library、PubMed、EMbase和Ovid,检索时间均从建库至2012年4月。纳入所有研究糖尿病与食管癌病因相关性的病例-对照研究。采用RevMan 5.0软件进行Meta分析。结果最终纳入6篇病例对照研究,其中病例组(食管癌患者)1 553例,对照组(非肿瘤患者)57 767例;两组暴露糖尿病患者分别为139例和2 294例。Meta分析结果显示,各研究间存在统计学异质性〔合并效应量OR=1.60,95%CI(1.13,2.26)〕。亚组分析显示,无论男性还是女性,病例组的糖尿病患病率均较对照组增高,且女性较男性糖尿病人群的OR值更高〔女性:OR=1.58,95%CI(1.14,2.18);男性:OR=1.41,95%CI(1.07,1.86)〕。结论糖尿病与食管癌存在关联,提示糖尿病可能是食管癌的一个独立致病危险因素。
