摘要：

IgA肾病(IgAN)是世界上最常见的原发性肾小球肾炎之一。既往认为IgA肾病是一种良性疾病,但近年发现30%～40%呈进展性,最终发展为终末期肾病,称为进展性IgA肾病。进展性IgA肾病的治疗方案应结合患者的临床和病理表现来制定:对于蛋白尿>0.5～1g.d-1的患者,首先应给予肾素血管紧张素系统(RAS)阻断药,以控制血压和最大程度地降低蛋白尿;3～6个月后蛋白尿仍持续≥1g.d-1,肾小球滤过率(GFR)>50mL.min-1.(1.73m2)-1,且病理表现以活动性病变为主的患者可给予糖皮质激素治疗;其他治疗药物和手段如免疫抑制药、抗血小板药物、扁桃体切除术等在延缓肾脏疾病进展中的价值仍需进一步评估。
