H-FABP在急性冠状动脉综合征早期诊断及预后评估中的价值

摘要
目的探讨心脏型脂肪酸结合蛋白(heart-type fatty acid binding protein,H-FABP)在急性冠状动脉综合征(acute coronary syndrome,ACS)早期诊断及预后评估中的价值。方法采用前瞻性研究对173例胸痛发作12 h内的患者分别检测H-FABP、肌钙蛋白I(troponin I,c Tn I)及肌红蛋白(myoglobin,MYO)、肌酸激酶(creatine kinase,CK)、肌酸激酶同工酶(creatine kinase-MB,CK-MB)等生化指标,并观察患者住院期间主要不良心脏事件(major adverse cardiac events,MACE)发生情况,收集、统计并分析相关资料。结果 1在胸痛发作0~3 h,H-FABP在急性心肌梗死(acute myocardial infarction,AMI)组的阳性率高于c Tn I、MYO、CK、CK-MB,其差异有统计学意义(P<0.05);H-FABP的敏感度、阳性预测值、准确度及Youden指数均高于c Tn I、MYO、CK、CK-MB。2在胸痛发作>3~6 h,H-FABP在AMI组的阳性率与c Tn I和MYO相似(P>0.05),但高于CK、CK-MB(P<0.05);H-FABP的敏感度、阳性预测值、准确度及Youden指数与c Tn I相似,但高于MYO、CK、CK-MB。3在胸痛发作>6~12 h,H-FABP的阳性率、敏感度与c Tn I相似(P>0.05),其阳性预测值、准确度及Youden指数与c Tn I、CK、CK-MB相似,但高于MYO。4在0~12 h内,H-FABP的特异度与c Tn I相似,但高于MYO、CK、CK-MB。5H-FABP在胸痛发作3 h内的诊断价值远远高于其他指标。6H-FABP、H-FABP+c Tn I的阴性预测值高于c Tn I、MYO、CK、CK-MB。7AMI组住院1周内MACE发生情况,H-FABP的OR值为2.761,95%CI为0.704~0.902。结论 H-FABP可作为早期诊断AMI的敏感指标,尤其在3 h内显著优于其他指标。c Tn I联合H-FABP在6 h内显著提高c Tn I的阴性预测值,可用于胸痛早期AMI排除诊断。H-FABP是AMI的MACE发生的危险因素。
