脑卒中后抑郁症临床对照及相关因素分析

摘要：

目的　探讨脑卒中后抑郁症的临床特征 ,并对相关因素进行分析 .方法　采用汉密尔顿 (HAMD)抑郁量表对 97例卒中后抑郁症和 78例内源性抑郁症的评分进行分析 ;使用CT或 (和 ) MRI确定病灶部位 ,并结合 HAMD抑郁量表的分值对病灶部位与抑郁的发生进行研究 .结果　抑郁症发生率为 33% .抑郁的发生与患者性别、病变性质无显著性差异(P>0 .0 5 ) .颅脑 CT或 (和 ) MRI示左侧半球比右侧半球、前半球比后半球更易发生抑郁 ,抑郁程度左侧重于右侧 ,前部重于后部 .卒中后抑郁症的焦虑 /躯体化症状发生高于内源性抑郁症 ,而认知障碍尤其是绝望感低于内源性抑郁症 .卒中后抑郁症的严重程度以轻中度者居多 (89.7% ) .结论　卒中后抑郁症的临床表现有一定特点 ,在治疗卒中同时应早发现并及时治疗 ,效果良好
