20例急性脑梗死患者神经康复治疗的功能磁共振研究

摘要：

目的探讨早期神经康复对脑梗死急性期患者脑功能重塑的影响。方法 40例脑梗死急性期患者(2周内)分为康复组(20例)和对照组(20例),康复组采用早期神经康复(入院24 h内开始)和药物治疗的方法,对照组仅采用药物治疗,疗程14d,比较两组治疗前后临床神经功能缺损程度评分以及日常生活活动能力(ADL)量表(Barthel指数)的评分,并通过血氧水平依赖-功能磁共振成像(BOLD-fMRI)观察两组患者脑功能重塑情况。结果康复组与对照组之间神经功能缺损程度评分改善值和ADL改善值t检验,差异均有统计学意义(P≤0.05),且康复组改善值明显大于对照组。康复组与对照组之间患侧脑运动皮质功能健存率改善值t检验,差异有统计学意义(P≤0.05),且康复组改善值明显大于对照组。结论早期康复能有效地促进急性期脑梗死患者神经功能恢复,能有效地促进患者脑功能重塑。
