类风湿关节炎流行病学进展

类风湿关节炎(RA)的患病率随着年龄增长而逐渐增加,65岁以上或70岁以上老年人患病率最高,文化程度较低者患病率较高,女性略高于男性。成年人RA患病率为0.2%~0.8%。RA危险因素:①遗传因素:RA患者家族研究显示一级亲属患病的危险性要比无亲属关系的个体高2~4倍,双生儿研究显示当同卵双生儿中有一方患RA,另一方患病率为12%~15%。②激素及生育因素:与男性相比,RA更常见于女性;妊娠期RA常可自发缓解。③营养因素:大量摄入烘焙或烤制的鱼类,特别是富含油脂的鱼类如鲑鱼和鳟鱼,可减缓RA进展。研究显示血清中含硒水平较高可防止RA发生。④其他因素:类风湿因子检测为阳性的个体、大量吸烟,可增加发生RA的危险性,同时也增加RA进展的危险性。临床RA患者具有较高的死亡率。RA患者生存期普遍缩短3~18年,并且每经过一个10年的跨度,死亡率的总体危险性就会增加1.3~2倍。与一般人群相比,RA患者更常死于感染和心血管疾病。
