经颈静脉肝内门体内支架分流术实验研究的初步报告

摘要
经颈静脉途径肝内门体内支架分流术(TIPSS)在近年已应用于临床,并取得良好疗效,但尚存在值得进一步研究的问题。本研究旨任探讨TIPSS的穿刺技术和肝内门体分流道的建立对健康动物肝功能的影响。实验动物为健康犬8只、猪2只,用Ross针经颈静脉、腔静脉、肝静脉穿刺门静脉,穿刺成功后用球囊导管扩张肝内穿刺道,然后植入内支架。术前及术后每间隔1个月抽静脉血检测肝功能,术后间隔4~6周造影复查。结果:TIPSS成功6只,植入长4.5cm支架7个(其中1只动物植入2个),支架直径8mm、10mm各植入3只动物;观察时间30天~95天,1只于术后30天分流道闭塞,其余保持通畅,TIPSS前、后肝功能指标无有意义变化。初步结论:TIPSS成功的关键是穿刺前了解肝静脉与门静脉的空间关系;在健康动物肝内建立直径8~10mm门体分流道,近期观察对肝功能无不良影响。
